About the Role

Pinellas County Urban League’s Case Managers create, implement, support, document progress and monitor Support Plans to assist their clients in meeting their individual and/or family economic goals. They use their experience and knowledge of the community and social services to assess needs and barriers, and provide interdisciplinary follow up and wrap around services.

Essential Duties & Responsibilities include but are not limited to:
• Assist in program recruitment, enrollments, and orientations
• Adhere to grant policies and procedures and meet program deliverables
• Conduct initial and ongoing assessments
• Help clients achieve wellness and self-sufficiency by crafting Individualized Support Plans (ISP) according to program requirements
• Monitor and document client on-going progress and/or program status, and adjust statuses based on changing needs and conditions
• Maintain caseload assignments
• Help clients make informed decisions by acting as their advocate regarding their ISP goals and options
• Maintain reliable and consistent contact with assigned clients
• Coordinate and provide holistic care that is safe, timely, effective, efficient, impartial, and client-centered
• Provide support service referrals and connect clients to other Pinellas County Urban League programs, as well as outside community resources
• Record and maintain accurate and timely case notes and reports
• Complete associated paperwork necessary to produce monthly impact reports
• Assist in providing on-going follow up/aftercare services to include but not limited to job retention
• Assist Job Developer with Host Agency placements
• Assess and address motivational and psychosocial issues, and make appropriate finding referrals
• Ability to learn government data entry system and other date collection systems
• Adhere to professional standards as outlined by the Pinellas County Urban League protocols, rules and regulations
• Attend weekly and monthly team and program meetings
• Maintain high standard of integrity and ethical conduct
• Complete other duties as assigned

Requirements

Required Education:

Bachelor’s Degree in Social Work or Human Services field and/or evidence of relevant work experience

  • Use Microsoft Outlook, Teams, SharePoint, Word, Excel, PowerPoint, etc. for various tasks

  •  Flexible schedule

  • Reliable transportation for occasional county to county travel
